Matt.Cross
I think some pics would be nice, barrel and extension, might not help us fix your problem but could be a great "don't let this happen to you" scenario.

82HALO
There is a very long thread about this happening to someone over on the ar forum. Many guys chimed in on this, and few gun Smiths...they all said it's pretty much toast! I would contact the manufacturer and see what you could work out! They also said the reaction rod and receiver clamp vise is a big no, no for this! Use a barrel clamp only! Tripledeuce
I would not worry to much about the bbl. Screw it back together, check the headspace, and if OK, then red locktight the hell out of the extension, and let it cure. I would be A LOT more worried about damage to the upper! Check it VERY carefully! Respectfully Terry
Robocop1051
Troy should take of you. I can't see why they wouldn't. A simple letter saying, "the barrel extension came off one of your barrels, would you please put it back on for me?"... should suffice. It'll be some down time though. I have a barrel clamp system and I rarely ever use it. It's really only needed for muzzle devices. I use crush washers, so there isn'the too much torque involved. Last time I used it, I lost the leather I usually put inside. I tried to use an old tshirt instead, and really messed up the finish on my barrel. 82HALO
These are what I use to clamp a barrel in the vise. www.amazon.com/gp/product/B0057PUEZO?psc=1&redirect=true&ref_=oh_aui_detailpage_o07_s00
Robocop1051
I have the old style aluminum barrel clamps. They allow me to do the heavy, m4 and pencil contour.
survivalshop
Barrel clamped into a Vice block of some sort is the only way to safely remove or install Muzzle Devices . That Barrel Extension should not have came lose with normal Torque , unless it wasn't tight to begin with , contact Troy before you try to re-install it . I thought Barrel Extensions were left hand threaded ? Maybe not all ? 150 ftlbs + on the Barrel extension . Its not all about if it HS 's correctly , it has to be correctly indexed to the Gas Port . You might get lucky & it tightened back to the correct spot .
Matt.Cross
I don't see where they ever installed the through pin to keep it from backing off the threads...
Robocop1051
The pin doesn't always go through. It's just for alignment.
slugger43
To reinstall you're gonna need one heck of a good barrel vise block set, and I don't know what the torque specs are for the barrel extension. I'd also say call the barrel manufacturer for their advice. If it were me, I'd have them do it. I'm guessing, but the extension may not have been torqued sufficiently when installed. I can't imagine a 6" wrench on a brake having enough torque. Man, you gotta be Mr. Gorilla arms!Barrel extension wrenches (reaction rods) are terrific for barrel installation, but should never be used to unscrew a muzzle device. kenichols29
I had the same issue with mine recently. I contacted the manufacturer and they replaced the barrel. I would contact them and see what they are willing to do.
