This is not my place to comment but... If you rack the bcg let's say 100 times the metal will mesh and the AR bolt will hone the edges of the extension a bit. Just Sayin' !

Welcome from AZ! Need a few more specifics , like is it new ? If so ... Rack the BCG About 100 times with some lube on it to mesh the upper to the bcg. When you go out next time take a DPMS metal mag with you and a different kind of ammo ,like 168 Gr projectiles. Wait for it.... Lube it up with Mobil One or Castrol GTX ! Dripping Wet, Running down your arm, Splashing in your face and dripping on the floor !!! This will break in your gun , 50 to 100 rounds . Run a patch through the barrel every 10 to 20 rounds. The 147 gr bullets are not the best to be used for breaking in your gun. Also the buffer is a little heavy. Magazine could be the other issue, try the metal mag with the 168gr bullet then the Pmag with the 168gr bullet. Good luck and tell us what happens.

Make sure you get high rings or a ring mount that gives you clearance.

.095" is a port size you can live with. Run a couple patches through the barrel, that should solve that issue. Just get your bcg heads paced before you shoot.

That picture looks like the gas block broke. You might want to get a new gas tube also and install the gas block and gas tube as a complete replacement unit. It will be easier that way. BTW ....Welcome from AZ !

If possible install a slip on .936" gas block with one or two hex nuts on the bottom.

Can you tell what type of primer ? Silver or bronze outside. Color of inside (paste). It's possible the primers are too strong for the powder charge.

I use heat guns ! Less stress on the metal.
