Susan worked with a lady that had her tubes tied when she had her 3rd son. Her husband had himself snipped as well. A couple of years later, she ended up pregnant by her husband with their 4th son. Both surgeries had reversed themselves.

How much lube did you have on the carrier? These tend to like lots of lube. Beyond that, what are the details of your build? Details are important. A parts list with brands can help to diagnose this. We may need measurements as well but we'll ask if needed.

I am at my grandma's old house. We had only been there an hour or so and hadn't had time to cool much. When we would come here when I was a kid, the excitement was sitting around and watching people sweat. 😰
